We are an inclusive disability support service that aims to strengthen, enrich, and empower the community of Sydney by providing a wide range of services to clients in their own homes or the community.

What Services Are Available?
- Individual support- personal care, Overnight care
- Medication management
- Carer service, Community access
- Youth support
- Social skills and relationships
- Disability employment assistance & education training
- Travel training & transport
- Disability – physical or intellectual – NDIS
- Home based rehabilitation assistance & Allied health assistance
- Meal preparation & Domestic assistance
- Yoga, adapted yoga (for people with higher support needs), meditation, yoga Nidra
Who can access Functional Foundations?
Everyone’s circumstances are different so you may be eligible to receive full or partial funding from the commonwealth Government through:
- NDIS (National Disability Insurance Scheme). We work with self-managed and plan managed clients
- Currently seeking approval to provide services for HCP (Home care package) and CHSP (Commonwealth Home Support Program)
Alternatively, Private fees for services are available for people who are not eligible for funding.
